Laparoscopic Resection of a Urachal-Sigmoid Fistula in a Heart Transplant Patient

چکیده
Introduction: Urachal remnants are rare in adulthood with only a few cases of urachal-sigmoid fistula reported in the literature. We report the first case of a laparoscopic resection of a urachalsigmoid fistula resulting from diverticulitis in an immunosuppressed heart transplant patient. Case Description: The patient is a 43-year-old male who underwent an orthotopic heart transplant one month prior for dilated cardiomyopathy. His presenting symptom was abdominal pain and imaging showed a sigmoid-urachal fistula. A laparoscopic sigmoidectomy with resection of the urachal cyst and drainage of pelvic abscess was performed. The cyst was found to be contiguous and inseparable from the bladder, and therefore a small cuff of bladder was included with the specimen followed by primary laparoscopic cystorrhaphy. He had an uncomplicated hospital course. Discussion: Our case contained several unique aspects that altered the ultimate care and hospital course of the patient. Immunosuppressed patients possess multiple risk factors predictive of poor surgical outcome—infection, bleeding, and hollow organ perforation. The patient in this study was on multiple immunosuppressants and exhibited no peritoneal signs despite an elevated leukocytosis and some worrisome radiologic findings. Our decision to operate early was influenced by the aforementioned factors and led to an uncomplicated recovery.

Infected Urachal Cyst Following Laparoscopic Cholecystectomy
Introduction: In an era of ultrasonography and computed tomography (CT), urachal remnants have been detected with increasing frequency. If these remnants become infected, they can mimic a variety of intra-abdominal pathologies. We present the case histories of two patients with an infected urachal cyst that developed after laparoscopic cholecystectomy and necessitated excision. Case Description...
متن کاملFactors Associated With Complications of Open Versus Laparoscopic Sigmoid Resection for Diverticulitis
BACKGROUND This study critically reviews sigmoid colon resection for diverticulitis comparing open and laparoscopic techniques. METHODS We conducted a retrospective review of all open and laparoscopic cases of diverticulitis between 1992 and 2001. Data analyzed included the following: indications for operation, postoperative complications, and incidence of laparoscopic conversion to laparotom...
متن کاملLaparoscopic Resection of Chronic Sigmoid Diverticulitis with Fistula
BACKGROUND AND OBJECTIVES A growing number of operations for sigmoid diverticulitis are being done laparoscopically. There is a paucity of data on the outcome of laparoscopy for sigmoid diverticulitis complicated by colonic fistula. The aim of this study was to compare the results of laparoscopic resection of sigmoid diverticulitis with and without colonic fistula. METHODS A retrospective rev...

often subject to infection, spontaneous rupture, fistula formation or a malignant change. Sigmoid-urachal fistulas have been reported in cases of colonic diverticulitis or an infected urachal cyst (1-3); however, for these cases, there is no communication with the urinary bladder or any association with urothelial carcinoma. Although the normal urachus is most commonly lined by the transitional...
متن کاملRetained pill camera at an entero-uracho-vesical fistula site in a patient with Crohn's disease.
An 18-year-old female patient with Crohn's disease had abdominal pain secondary to a retained pill camera. After several weeks of medical management, the camera spontaneously passed. However, the patient also had an intra-abdominal abscess that worsened, despite medical therapy. Surgical therapy was recommended and a 5cm infected urachal cyst with entero-urachal and vesico-urachal fistulas was ...
